By making a booking, the customer agrees to be bound by our Terms & Conditions.
We reserve the right to change, update or reissue these Terms and Conditions at any time. Any such changes will be posted on our website and the customer agrees to be bound by such changes.
References to “we”, “us” and/or “our” in these Terms and Conditions is a reference to Physio Limited.
SERVICES
Here at So Physio we believe in providing the best clinical care and customer service to our clients. As health service providers, we comply to the Code of Health and Disability Services Consumers’ Rights and assure that our clients have the right to decline part or all of the treatment offered at the any time, to seek a second opinion, or to have a chaperone/support person present if needed. We provide Physiotherapy, Acupuncture, Clinical Pilates, Neuro Kinetic Therapy as part of our specialised services.
APPOINTMENTS
So Physio is an ACC registered clinic and our providers are ACC registered practitioners under the Accident Compensation Act 2001. We also provide non-ACC services. Our initial appointments are 1-hour appointments to ensure that we have a discussion, perform a full objective examination, evaluate your needs, provide you with a diagnosis and developing a treatment programme. Following treatment sessions will either be 30-minutes or 1-hour depending on what is required. Your physiotherapist reserves the right to recommend the time required for following sessions, however, if time or cost is an issue, we would be happy to discuss and work around it.
CONSENT
Your practitioner will provide you with a consent form to fill during your first session as well as a copy of these terms and conditions to read before signing.
PRIVACY POLICY
We treat all personal and health information supplied by you as confidential in accordance to the Privacy Act 1993 and Health Information Privacy Code 1994. We will not share your information to any third party without your prior permission. Please refer to our full Privacy Policy here.
PAYMENT POLICY
Payment is expected at the time of consultation. A $15 administration fee will be added to your account if it is not paid at the time of the consultation. This fee will be removed if the account is paid within the next 7 days. Full payment prior to any Telehealth appointment is expected. We accept cash, EFTPOS, payment through Internet Banking and easy claims through Southern Cross.
OVERDUE ACCOUNTS & OUTSTANDING DEBTS
Overdue accounts that have not been paid in 30 days will be passed onto the collection agency BayCorp. This will result in an additional administrative fee of $30 and the account holder of the account will be liable for any costs incurred by the Collection Agency on outstanding debt. Collection Agency costs will be charged at their service rate plus GST.
CANCELLATION
We enforce a strict cancellation policy at So Physio in order to be fair to every client wanting an appointment.
· If you cancel or do not turn up for a session
We require a 24-hour notice period. If you have booked a session and cancel within that time frame or do not turn up for the session, then you are liable to pay the cost of that session.
· If we cancel
On the occasion that we may have to cancel a scheduled session due to sickness or other emergencies, we will book you another appointment as soon as reasonably possible. You will not be charged a cancellation fee if the session is cancelled by us.
LATENESS
If you are late arriving at a session, the session will begin on your arrival and continue until the time when it is scheduled to end. You are obliged to pay for the length of time the session was originally booked for.
COST & PAYMENT
The cost of each session is confirmed by your practitioner to you. We have a surcharge for any ACC services provided. In the event of any pricing changes, we will inform you as soon as possible. We accept payment in cash, credit, debit, EFTPOS or through internet transfer. If you require a receipt, please inform your practitioner and this will be arranged promptly. Any prepayments & credits are only valid for ONE YEAR from payment and are non-transferrable to other types of treatment besides the one paid for.
We also reserve the right to change the pricing of our services accordingly, without advance notice to the customer. Any changes in pricing will be updated on our website and on Cliniko.
LOYALTY & REFERRAL PROGRAMMES
Any discount given out as part of our referral programme is only valid for new customers only.
